With Critical Alert’s online RMA Center, you can request an RMA 24 hours a day, 7 days a week. Critical Alert offers repair services for both warranty and out-of-warranty products.
To begin the process, simply complete the electronic form below by providing the required information. Upon approval, Critical Alert will issue an RMA number, along with shipping instructions, to the customer via email within 24 hours of receiving the request ticket. All RMA numbers are valid for 30 days. If the product is not received within 30 days a new RMA number must be requested.
When you are ready to ship the product to Critical Alert for service/repair, you will need to include a copy of the RMA in the shipping package. You should also note the RMA# in the Attention Line of the address. Please keep in mind that the customer is responsible for all shipping to and from Critical Alert at the address below.

– Purchase Order Number
– Shipping Information
– Model Number (MDL)
– Serial Number
– Brief Description of failure
